If you need to carry a medical device or have a medical condition, please inform your Wellness teacher.  It should also be brought outside for class.
Locker room:
Students are to be in the locker room when the bell rings. ID’s are to be visible, around the neck to enter the locker room.  After the bell, there will be an additional 4 minutes allowed to change and line up for attendance. The locker room doors will remain locked after all students are out.  No student is to enter once the doors are locked.  It is the student’s responsibility to bring a combination lock to use during class.  Locks should be removed daily after each class.  Back packs, cell phones/Ipods and all electronic devices are not allowed outside of the locker room.  Proviso West is not responsible for lost or stolen items.  Please lock all valuables!
Grading:
Each student will receive 10 points per class for proper dress and participation. Tardies will result in a 2 point loss.   In the event that a student does not dress, the student will be required to write or participate.  Inappropriate shoes or jeans will automatically result in a NO DRESS.    During a NO DRESS, the student will be required to leave their back pack in the locker room and they will be required to participate or write a Full Page during the entire class.  In the event of a medical excuse, the student will be required to complete written work at the discretion of the Wellness teacher.  Points will also be earned through fitness testing and other select activities.  It is important that you dress daily in order to receive points for a passing grade.  Final exams will be given in all Lifetime Fitness classes.  This exam will count as 20% of the grade.  Each quarter grade will count as 40% of the grade.  The remaining portion of your wellness grade will be in the form of written reflections or a video analysis (found on my website) that will be due on a bi-monthly basis.
District 209 adheres to the state policy of being dressed and prepared for activity in Physical Education class.  It is required that if you are not participating because of lack of preparation (or can’t participate due to personal reasons, injury, etc.) you must complete a written assignment.  Include name, hour, and why you sat out.
